Pre-Admission
An admission assessment will be completed prior to admission by the Director of Nursing and the Admission Director in coordination with the facility Administrator in order to find the best possible placement for the resident.
The Director of Nursing and the Admission Director will be reviewing previous and current medical diagnosis, along with current medications. The resident needs to have a current history and physical at the time of assessment. If the patient's physician will not follow at Allison Care Center, the facility will assist the resident in finding a physician.
Preparing for your stay at Allison
Prior to admission, to help facilitate a smooth admittance for you or your loved one to the facility, a number of forms must be completed. Listed is information and items that will need to accompany you to assist in expediting the admission process.

Understand your payor source (Private pay, Medicare, Medicaid, Medicaid pending, or an HMO).

Current medical and physical history.

Current medication list.

Power of Attorney, Proxy forms and/or related documentation, such as Guardianship or Conservatorship papers.

Insurance information and cards such as Medicare, Medicaid, and HMO's.

Social Security Card.

- Living Will
- Durable Power of Attorney
- Do Not Resuscitate Form

Responsible party, emergency contact telephone numbers and addresses.

Mark/identify all personal belongings prior to admission with a permanent marker or stitching.

Bring all food (in airtight containers), all medication (including over the counter items), and personal items to the nurse's station for check-in.

Bring additional spending money to the business office for safe keeping.
