Welcome to Clear Creek Care Center of Westminster. Located in a quiet residential neighborhood, our care and services have been an integral part of the community for over 30 years.

Clear Creek Care Center provides extended care and short-term rehabilitative services. Our separate Transitional Care Unit  is an ideal setting to provide specialized services for individuals needing medical care and rehabilitation following a hospitalization, or a medical procedure. Our interdisciplinary team offers patient-centered plans of care to focus on the goals and needs of each individual.

Residents can enjoy the comfortable surroundings that our care center offers. This includes a gift shop, garden room and many outside amenities.
