Eagle Ridge of Grand Valley is a small, privately owned facility, located in a residential community setting. We have been an integral part of the community for many years. We strive to provide a facility that is warm, inviting and homelike, providing a comfortable setting for our residents.
At Eagle Ridge of Grand Valley, our goal is to deliver compassionate care specifically geared toward the individual needs of you or your loved one. We offer 24-hour skilled nursing services and a comprehensive rehabilitation program that includes physical, occupational and speech therapy. Each resident receives on-going assessments from our highly trained and caring interdisciplinary team members. A personalized plan of care is created for each resident. Family members are encouraged to take an active role in care planning sessions. We offer a broad spectrum of healthcare services, including:

Family involvement is encouraged at our facility. We urge our residents’ families to visit as often as they are able. We do not have “visiting hours,” and friends and family are always welcome, provided they do not interfere with our residents’ privacy and sleep requirements. We welcome Medicare, Medicaid, and most managed care plans, private insurance programs and private funds.
