• Maxine Wilson - Nursing Home Administrator

  Maxine Wilson is our Nursing Home Administrator. She is an RN, and is a certified Nursing Home Administrator. She has been at Eagle Ridge for 1 year and 7 months, and has worked in health care for 19 years. Maxine states, “My goal as an administrator is to have a building that residents and families feel is a home for their loved one. I want our employees to love to come to work and know they make a difference in the lives they touch.” She is married to her husband, Ron, and they have two children, Wendy and Alan, and one granddaughter, Shelby. In her spare time, Maxine says, “I love the outdoors, and outdoor sports-hiking, camping, fishing, 4-wheeling, and snowmobiling. I also like to quilt and read.”

• Barb Sakala and Jennifer Cornwell - Director and Assistant Director of Nursing

  Barbara Sakala is our Director of Nursing. She is a Registered Nurse (RN) and is a certified Hospice and Palliative Nurse. She has been working for Eagle Ridge for 18 months, but has been a nurse for 28 years. She has worked in Long Term Care, as well as in Hospice, and has been both in management, and worked as a nurse on the floor. Her goal as the Director of Nursing Services is to look at quality of life of the residents. “I want to make sure the residents utilize their energy for the things that are most of importance to them. I also want to provide adequate symptom management before and at the end of life based on resident choice and honor those decisions.” She is married and has 5 children and 8 grandchildren. She also has 3 Labradors, a flock of chickens and 3 turkeys. In her spare time, Barb enjoys studying the art of the Anasazi and ancient Fremont Indians. She also enjoys hiking into ruins and photographing her experiences. She also enjoys gardening, artwork, painting and beadwork.

  Jennifer Cornwell is our Assistant Director of Nursing. She received her RN degree through Colorado Mesa University. She has been working at Eagle Ridge since August of 2011. Jen has worked in Long Term Care for almost 4 years. She says, “I enjoy working in this environment and have truly realized how important it is to take care of our elderly population.” Jen has two beautiful children-a 16 year old son Conner, and an 11 year old daughter Christiana. They also have a 3 year old German Sheppard named Belle, who “I guess you could consider a 3rd child.” In her spare time, Jen enjoys reading, hiking and spending time with her kids, and loves to cook. “I try to do as much of that [cook] as I can.”

• Carolyn Pacheco - Social Service Director

  Carolyn Pacheco is our Social Service Director. She has a Bachelor of Arts in Sociology, Human Service Emphasis with a Psychology minor, and is presently pursuing a master’s degree. She has worked for Eagle Ridge for a little over 14 months, but has almost 20 years in Human Services including at risk youth, individuals with intellectual disabilities, individuals with major mental illness, and geriatrics. Carolyn says, “My goal as a social worker is to make a difference in someone’s life every single day.” Carolyn is in a long-term relationship, and her 17 year old niece lives with her. “I have shared custody of a jack Russell terrier, and have two very spoiled cats,” she says. In her spare time, she enjoys reading, cooking, hiking, camping, collecting Day of the Dead art, shopping and traveling.

• Christine Lester - Dietary Supervisor

  Chris Lester is our Dietary Supervisor. She has been at Eagle Ridge for about 15 months. She has her Associates of Business degree, and has worked in the healthcare industry for over 18 years. Chris says: “My goal as a dietary supervisor is to allow the resident to make choices in what they eat, and to give the best quality of food and service.” In her spare time, Chris loves to read, likes to travel and is an avid Coca Cola collector.

• Damion Smuin - MDS Coordinator

  Damion Smuin is our MDS Coordinator, and has his LPN. He has been working at Eagle Ridge since August of 2010, and has been in health care since December of 1992. Damion says, “My goal is to help assemble and coach a team that understands the importance of making our residents transition through the later years of their lives as comfortably as possible.” He is NOT married, and does not have pets, but he DOES have 2 amazing kids: Mikel (19) and McKenzie (18). In his spare time, Damion enjoys to travel, scour the country for early American farmhouse antiques, garden, and entertain.

  Daryl Dord Hampton is our Director of Environmental Services. He has worked for Eagle Ridge since January of 2007, and this is his only experience working in health care. Daryl says, “My goal is that all residents are comfortable and safe.”

• Kimberly Meuret - Therapy Manager

  Kimberly Meuret is our Therapy Manager. She is a Certified Occupational Therapy Assistant, and has been working at Eagle Ridge for 4 years. She worked at an Assisted Living facility in Minnesota for 5 years prior to moving to Colorado in 1997, and has worked with Therapy since. Kim says, “My goal is to help residents reach their maximum potential to return home or to make Eagle Ridge their home.” Kim is married for 4 years, and has 2 children, 5 cats, and 1 dog.

• Patty Herold - Medical Records Director

  Patty Herold is our Medical Records Director. She is licensed in Emergency Medicine, and has been at Eagle Ridge since December of 2009. She has a varied background in healthcare. She explains, “I started in the medical field as an EMT in and firefighter in 1977. I have been a police dispatcher, task force manager for abused children, collections/credit manager at a hospital, and began in Long Term Care in 1988 as an Administrative Assistant. I have also worked as a Purchasing agent, and I have been doing Medical Records since 1990. I was the corporate medical records trainer for Western Colorado, and trained new Medical Records personnel throughout the valley.” Her goal as Medical Records Director “is to make sure that our residents’ medical records are always organized, up to date, and complete. Medical Professionals depend on their patient records to be complete and correct, so that they can care for their patients in the best professional manner available. The medical record tells a story about our residents lives and is a sacred part of their elder years. I am their paper ‘caretaker.’” Patty says, “I am married to my best friend of 18 years. I have 5 sons, all of whom are grown with children of their own. I have 13 grandkids that range in age from 2 months to 19 years.” In her spare time, Patty says, “I own a Photography Company and my husband and I are always out and about taking pictures in our spare time. We also own a construction company which keeps us busy. And we are involved in a nutritional supplement company. Our leisure time (when we have any) involves boating, 4 wheeling, fishing, and spending time outdoors. I love to read and facebook my kids and grandkids as a way to keep in touch with them. My favorite past time is spending time with my grandkids teaching them the wonders of our great big world.”

• Sherri Suazo - Director of Housekeeping and Central Supply

  Sherri Suazo is our Director of Housekeeping/Central Supply. She has worked at Eagle Ridge for 6 years, and she has been a supervisor for 13 years. She has worked in the health care industry for 15 years. Her goal is, “Resident care and to make sure our building is clean and safe.” She has two wonderful kids, and 9 awesome grandchildren, and a boyfriend of 13 years. In her spare time, Sherri enjoys cooking and riding motorcycles and all outdoor sports.

  Tina Sparks is our Business Office Manager, and has been working at Eagle Ridge since July of 2011. She has been in the health care industry for 19 years. Tina says, “My goal to is to make sure my job duties are taken care of the best I can and do what is expected of me.” She has been married for 22 years, and has two kids, Jaron (a sophomore at CMU ) & Kaisha Senior at CHS. She has 2 dogs..Zoey and Lucky, and one kitty cat, Dutchess. In her spare time, she says, “I spend my spare time running Booster Club for Central High school. That includes working / running Bingo sessions every Saturday night at Gold Mine Bingo. I also enjoy the sports Football , Wrestling and Golf.”

• Wendy Berryman - Admissions and Marketing Coordinator

  Wendy Berryman is our Admissions and Marketing Coordinator. She has been at Eagle Ridge since May of 2010, but has been in health care for about eight years. She has worked as a C.N.A., a medical receptionist, a Social Services director, and now as admissions and marketing director. Wendy says, “My goal is for families and residents to feel at home and that their needs are being met. Eagle Ridge is the resident’s home for however long they are here, and I want to be able to make the transition from home to long term care as easy on them as possible, and ensure that their needs are being met.” Wendy has been married to her husband Tod for two years, and they have two dogs, Sheeba and Pixie. Wendy says (about her spare time) “In the summer I go four wheeling with my husband, and our parents. I love to travel-especially to the ocean. In the winter, we go snowmobiling, watch football, and occasionally go to the movies. I love to scrapbook, read, quilt, and go to antique stores. Soon I will be starting school to finish my bachelor’s degree.”
