Admissions

Pre-Admission

A facility representative will complete an assessment before admission to assure that we at Westwind can meet all of your needs. The admission team will review your current medical diagnoses, current medications and functioning status. The current primary care physician will complete a history and physical at the time of admission. If you do not have a primary care physician that will follow we will assist you in finding one.

Admission Checklist

• Admission Packet
• Insurance information and cards such as Medicare, Medicaid, HMO
• Resident's birth Date and place
• Physician orders (if coming from home)
• History and physical from current physical
• Advanced Directives: Living Will, Durable Power of Attorney
• (Do Not Resuscitate FOrm)
• Social Security Card
• Pharmacy
• Responsible party address and telephone number
• Guardianship / Conservatory papers
• Personal Items

Some items will be necessary to meet the resident's needs. All personal items should be marked with the resident's name in permanent marker or etched in.

• Comfortable, machine washable clothing
• Eyeglasses
• Dentures
• Hearing aids
